Chronic Disease Management

A chronic medical condition is one that has been present for six months or longer, for example, asthma, cancer, heart disease, diabetes, arthritis, stroke and epilepsy and other chronic conditions.

The GP Management Plan is a written set of goals to help patients manage their chronic condition. A GP Management Plan involves your doctor and practice nurse developing a plan of management in consultation with you. The GP Management plan is designed for patients who require a structured approach whilst enabling the doctor to plan and co-ordinate the care of patients with complex conditions requiring ongoing support from an Allied Health team for example physio, podiatrist, optometrist, psychologist etc. A GP management plan aims to to ensure everyone involved in your care works together to help you manage your condition.

Our doctors will suggest a consultation with one of our nurses for this plan to organise 5 sessions with allied health professionals for free.

This service is free of charge and the appointment will take approximately 45 minutes. Please be aware that if you require seeing the doctor on the same day as the practice nurse, there will be additional charges due to the new changes in Medicare as of 1st of July 2014.
